Discharge Care

Returning home from hospital can be overwhelming both for the individual and for the family. There are new medications, new risks, and often, a new way of moving through the world. Our post discharge care is designed to make that transition feel seamless, supported, and safe. We can start before the discharge itself, coordinating with medical teams, preparing the home, and ensuring nothing is left to chance. Once home, we provide a calm, steady presence: assisting with mobility, care, nutrition, and emotional support.
